A thicker muscle on a scan, a stronger contraction and an easier everyday movement are three different results. Knowing which one a study measured makes its findings more useful.
Size and shape
MRI or ultrasound can measure structures such as muscle thickness, cross-sectional area or a layer of subcutaneous fat. The unit and location matter. A percentage change at a selected scan location is not automatically a percentage change in whole-body muscle or fat.
Strength
Strength is the ability to produce force. It can be assessed using an appropriate strength test. A scan can show structural change, but it does not directly measure the force someone produces.
Function
Functional tests ask what a person can do: for example, walking, standing from a chair or completing a timed task. These outcomes can be influenced by several systems, not only one muscle.
The patient's experience
Questionnaires can measure symptoms, activity difficulty or quality of life. These outcomes can be meaningful, but a percentage improvement in a score is not the same as that percentage of people becoming symptom-free.
Where does body weight fit?
Body weight combines many components. Localised contour changes can occur without substantial weight loss. Conversely, a lower number on the scales does not identify which tissue changed. Who was studied? What treatment and comparison were used? What was measured? When was it measured? What remains uncertain?
