At ¹EM, the standard initial EMSELLA® treatment course is six sessions, usually scheduled twice weekly over approximately three weeks.
Initial course, review and maintenance
An initial course, an assessment of response and any later maintenance are separate decisions. A package size does not establish that extra sessions are needed or suitable. The clinician checks the relevant treatment instructions and your response before recommending further treatment.
This is also the typical treatment schedule recommended by BTL, the manufacturer of EMSELLA®, and it is the protocol used in multiple published clinical studies evaluating the treatment in both women and men. [1-3]
Each session takes approximately 28 to 30 minutes.
Your treatment is still personalised. Dr Emir will assess your symptoms, medical history and goals before treatment and review how you are progressing through the program.
For most patients, however, the simple answer is:
6 sessions
2 sessions per week
Approximately 3 weeks
Why are six EMSELLA sessions recommended?
EMSELLA is designed as a course of pelvic floor muscle stimulation, rather than a one-off treatment.
During each session, high-intensity focused electromagnetic energy stimulates repeated contractions of the pelvic floor muscles.
Just as other forms of muscle conditioning depend on repeated stimulation over time, the aim of an EMSELLA® course is to progressively stimulate and strengthen the pelvic floor across multiple sessions.
This is why both the manufacturer and many published clinical studies use a series of treatments rather than assessing a single isolated session. [1-3]
Is six sessions an evidence-based treatment schedule?
Yes.
A recent clinical study involving women with stress and mixed urinary incontinence used:
6 sessions
twice weekly
approximately 28 minutes per treatment
The study reported significant improvements in urinary incontinence scores and pad use after the six-session course, with further improvement observed at one-month follow-up. [2]
A 2025 study evaluating EMSELLA® in men with persistent urinary incontinence following radical prostatectomy used the same schedule:
6 sessions
twice weekly
28 minutes per session
Participants experienced significant improvements in urinary symptoms and pad use following treatment and at one-month follow-up. [3]
Another study of women with stress urinary incontinence also used six HIFEM treatments scheduled twice weekly and reported improvements that were assessed for up to six months. [4]
This is why six sessions is a useful evidence-supported starting course rather than an arbitrary package size.
How often should I have EMSELLA?
Usually twice per week.
This allows the six-session course to be completed over approximately three weeks.
For example, your appointments might be scheduled:
Week 1: Session 1 and Session 2
Week 2: Session 3 and Session 4
Week 3: Session 5 and Session 6
The exact appointment days can be arranged around clinic availability and your schedule.
If there is a clinical reason to alter the timing, your treatment schedule can be reviewed.
Do the sessions need to be on consecutive days?
No.
A typical EMSELLA program is distributed across the week rather than requiring treatment every day.
The standard schedule of twice-weekly appointments also makes the program relatively easy to fit around work and normal daily activities.
How long is each EMSELLA session?
A typical treatment takes approximately 28 to 30 minutes.
You remain fully clothed and seated on the EMSELLA® chair throughout treatment.
There are:
no injections
no internal probes
no surgery
no anaesthetic
You can generally return to your normal activities immediately afterwards.
This means the entire standard initial treatment course involves approximately three hours of actual treatment time spread over three weeks.
Will one EMSELLA session be enough?
A single session is not the standard treatment course.
Some people may notice changes early in their program, and the manufacturer notes that some patients report improvement after their first treatment. [1]
However, EMSELLA® is intended to be delivered as a series of sessions.
The research supporting the treatment generally evaluates outcomes after completing a treatment course rather than assuming one session provides the full effect. [2-4]
For that reason, ¹EM recommends assessing EMSELLA as a structured treatment program rather than judging it solely on the first session.
When might I start noticing results?
This varies.
Some patients notice a change during their course, while published studies have also demonstrated continued improvement after the final treatment session. [2,3]
For urinary incontinence, changes might include:
fewer leakage episodes
reduced pad use
less leaking with coughing or sneezing
greater confidence during exercise
improved ability to control urgency
improved confidence in daily activities
Pelvic floor strengthening is a process.
The most useful question is therefore not simply:
"Did I feel different after one session?"
It is:
"How have my symptoms and pelvic floor function changed across the treatment course?"
Can results continue improving after session six?
Yes, they may.
Clinical studies have reported improvements continuing after completion of the initial six-session program.
In one study of women with stress and mixed urinary incontinence, symptom scores improved after the sixth treatment and improved further at the one-month follow-up. [2]
The 2025 male post-prostatectomy study showed a similar pattern, with urinary incontinence scores improving after six treatments and improving further at one month. [3]
This is one reason we do not assume that the final treatment day represents the final result.
Does everyone need exactly six sessions?
Six sessions is the standard initial course at ¹EM, but treatment still starts with an individual assessment.
Your clinical context matters.
Dr Emir may consider factors including:
the reason for treatment
type of urinary symptoms
symptom severity
pelvic floor weakness
postpartum history
previous prostate treatment
previous pelvic surgery
medical history
treatment response
your goals
The initial course provides a structured treatment period after which progress can be assessed.
Do men also usually have six sessions?
Yes.
A six-session schedule has been used in published EMSELLA® research involving men, including men experiencing persistent urinary incontinence following radical prostatectomy. [3]
The number of sessions may be similar, but the clinical reason for treatment can be very different.
A man undergoing pelvic floor rehabilitation following prostate surgery should not simply be treated as though he has the same presentation as a woman experiencing stress urinary incontinence after childbirth.
At ¹EM, the clinical assessment and goals are personalised even when the treatment schedule is similar.
What about EMSELLA after pregnancy or childbirth?
For suitable postpartum patients, a six-session EMSELLA® course may also be recommended after appropriate assessment.
At ¹EM, EMSELLA® is not performed during pregnancy or within the first three months postpartum.
After three months, Dr Emir assesses recovery, delivery history, symptoms and treatment suitability before recommending a program.
What if I am having EMSELLA for erectile dysfunction or men's pelvic health?
The treatment schedule may still begin with a structured course, but the clinical goal is different.
For selected men, pelvic floor strengthening may be relevant to:
urinary control
post-prostate rehabilitation
pelvic floor weakness
aspects of erectile function
Dr Emir will assess the reason for treatment before recommending the program.
What happens after the sixth session?
We review progress.
Completing session six does not mean the treatment process should become automatic.
Depending on why you are having treatment, progress might be assessed using:
frequency of urinary leakage
severity of leakage
pad use
urgency
urinary frequency
confidence during exercise
functional improvement
changes in the symptom that originally brought you to ¹EM
The purpose is to determine what has actually changed.
Will I automatically need more than six sessions?
Not necessarily.
¹EM does not assume that every patient automatically needs continuous treatment after the initial course.
After your program, the appropriate next step depends on your response.
That may mean:
no immediate further treatment
continued observation
pelvic floor exercises or another rehabilitation strategy
reassessment
additional treatment where clinically appropriate
If symptoms persist, change or later return, the first step is reviewing what is happening rather than automatically purchasing another course.
Will I need maintenance sessions?
Some patients may eventually consider additional treatment, but maintenance should be individualised rather than automatically prescribed to everyone.
Pelvic floor function can be influenced over time by:
ageing
pregnancy
childbirth
prostate treatment
weight change
physical conditioning
medical conditions
pelvic surgery
adherence to pelvic floor exercises
If further treatment may be useful, this can be discussed after your response to the initial course is known.
What if I miss one of my six appointments?
Contact the ¹EM team.
Missing an appointment does not mean you should simply abandon the entire program.
We can help reschedule the session and keep your course moving as consistently as possible.
Where possible, let us know early so another suitable appointment can be arranged.
Can I have the treatments more quickly to finish sooner?
The standard ¹EM program is designed around the established twice-weekly schedule.
There is little benefit in trying to compress treatment simply for convenience without considering the treatment protocol.
If you have travel, work or another scheduling issue, discuss it with the team and we can determine the most appropriate appointment arrangement.
Do I need a consultation before starting the six sessions?
Yes.
Before treatment begins, ¹EM assesses:
what you are hoping to improve
your symptoms
relevant medical history
previous procedures
implanted devices
treatment contraindications
whether EMSELLA® is actually appropriate for your presentation
EMSELLA is non-invasive, but it is still a medical device with important contraindications.
The six-session program should therefore follow assessment rather than replace it.
What if I have a pacemaker or metal implant?
Tell us before treatment.
EMSELLA uses a strong electromagnetic field and is not appropriate for patients with certain electronic or metallic implants.
A pacemaker is a contraindication.
Other implants require appropriate assessment based on the specific device, material and location.
An online answer or reception should not independently clear an implant for treatment.
Frequently asked questions
How many EMSELLA treatments do I need?
The standard initial course at ¹EM is six treatments.
How often are the treatments?
Usually twice per week.
How long does the full EMSELLA course take?
Approximately three weeks when treatments are scheduled twice weekly.
How long is each session?
Approximately 28 to 30 minutes.
Is one session enough?
One session is not considered the standard treatment course. EMSELLA® is designed to be delivered as a series of treatments.
Can I notice improvement before session six?
Some patients may notice changes during the treatment course, but individual response varies. Published research typically assesses outcomes after completion of a treatment series and at subsequent follow-up.
Do women and men have the same number of sessions?
Six sessions is commonly used for both women and men, although the reason for treatment and individual clinical plan may differ.
Do I automatically need another six sessions afterwards?
No.
Progress should be reviewed before deciding whether any additional treatment is appropriate.
Do I need maintenance treatment?
Not everyone does. Any future treatment should be based on your symptoms, goals and response to the initial program.
What if I miss an appointment?
Contact the clinic and we can help reschedule your session.
Do I need a referral?
No referral is required to request a consultation at ¹EM.
Six sessions. Three weeks. A personalised plan.
For most patients considering EMSELLA®, the treatment schedule is straightforward:
Six sessions, usually twice weekly over approximately three weeks.
What matters just as much as the number of sessions is making sure the treatment is being used for the right problem, in the right patient, with a clear way of assessing progress.
That is why the ¹EM process starts with assessment and continues through review rather than simply selling isolated chair sessions.
