Bladder control is a coordinated system. Your brain, bladder, nerves and pelvic-floor muscles work together to store urine and release it at an appropriate time. Strength is one part of that system; timing and control matter too.
What happens as the bladder fills?
The bladder stores urine. Signals travelling through the nervous system tell the brain about filling. During storage, the outlet and pelvic-floor muscles help maintain continence. Passing urine requires coordinated bladder contraction and relaxation of the outlet. This is a simplified explanation of a more complex control system.
Why can an urge feel so sudden?
Urgency is a sudden compelling need to pass urine that is difficult to defer. It can occur with or without leakage. Leakage during a cough, jump or lift is a different pattern, called stress incontinence. Some people have both. The symptom pattern helps guide assessment; it does not identify the cause by itself.
Where does EMSELLA fit?
EMSELLA uses electromagnetic stimulation to activate pelvic-floor muscles while you remain seated and clothed. BTL's educational seminar describes this as pelvic-floor re-education, including the brain–muscle connection. That is a useful way to discuss activation and control. It should not be interpreted as proof that treatment permanently rewires the brain.
Clinical research has reported improvements in urinary symptoms following HIFEM courses. A 2026 prospective study included stress, mixed and urgency incontinence, with 57 participants completing treatment. Urgency cases formed a small part of that uncontrolled study, so its overall result is not an urgency-specific success rate.

Bladder training and pelvic-floor rehabilitation address different parts of control and can be considered alongside other treatment options. A clinician can help identify the right approach, rather than assuming every bladder symptom needs more strengthening.
New painful urination, blood in the urine or difficulty emptying needs medical assessment. Inability to pass urine needs urgent care.
